As a Case Manager, you¿ll be a vital member of the patient-centered care team, responsible for identifying, assessing, planning, and coordinating care to meet the health needs of individuals and families. Your role ensures safe transitions between care levels, promotes patient safety, and optimizes health outcomes along the care continuum.

What You Will Do

• Collaborate with the care team to identify and assess patient needs, developing individualized care plans.
• Coordinate services and resources to ensure a safe transition to the next level of care.
• Advocate for patients and families, addressing social determinants of health and connecting them with community resources.
• Evaluate care outcomes and adjust plans to promote patient safety, quality, and cost-effective results.
• Facilitate communication between patients, families, and healthcare providers to ensure comprehensive care.

Qualifications

What You Will Need

Required Qualifications

• Bachelor¿s degree in nursing or Master¿s degree in Social Work.
• Minimum 3 years of clinical experience.
• Registered Nurse (RN) or Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license.
• Excellent verbal communication, written documentation, and interpersonal skills.
• Strong problem-solving, analytical, and decision-making abilities.
• Proficiency in computer skills and knowledge of healthcare systems.

Preferred Qualifications

• Certification in Case Management (CCM) or Accredited Case Manager (ACM).
• Experience in discharge planning, home health care, rehabilitative medicine, or managed care.
