About the role

Te whiwhinga mahi: The opportunity

Are you passionate about championing Māori health equity and making a real difference in the lives of whānau? We are looking for a Community Care Coordinator to join our innovative, cross-agency partnership in the Tairāwhiti region.

This is a unique opportunity to work across Pinnacle Midlands Health Network, Te Rūnanga o Tūranganui a Kiwa, and Ngāti Porou Oranga, connecting whānau to the health and social services they need to thrive. Rather than sitting within a single team, you will work fluidly across the health sector, building relationships and coordinating wrap-around care.

In this role you will:

  • Champion Māori health and equity, prioritising whānau with the greatest need.
  • Coordinate care across health and social services for individuals, whānau, and communities.
  • Facilitate smooth transitions and continuity of care across providers.
  • Identify and support those at greatest risk of poor health outcomes.
  • Act as the voice of primary health within the partnership, supporting Iwi aspirations.
  • Work collaboratively across agencies to deliver whānau-centred care.

This is a full-time (40 hours per week) fixed-term role until 30 June 2027.

He kōrero mōu: About you

  • Registration as a health professional (e.g. registered nurse, physiotherapist, occupational therapist, social worker) with a current annual practising certificate.
  • Primary or community healthcare experience.
  • A strong understanding of health inequities in Aotearoa New Zealand and a commitment to achieving equitable outcomes for Māori.
  • Excellent communication, time management, and organisational skills.
  • Commitment to Tikanga Māori and Te Tiriti o Waitangi.
  • A current full driver’s licence.

Ngā āhuatanga kei a mātou: What we offer

Shared values of:

  • Whakawhanaungatanga (connection) - through relationships we develop understanding.
  • Akoranga (learning) - we invest heavily in the continued learning and growth of our tangata (people).
  • Mahi tahi (collaboration and partnership) – we passionately pursue our purpose of making it easier for people to stay healthy and achieve equitable health outcomes for all. We work in collaboration and partnership with general practice, hapū, iwi, other health organisations and some of our services include smoking cessation, screening, mobile immunisation, before school check, extended care and school-based health services.
  • Kawa whakaruruhau (cultural responsiveness) - we strive to uphold Te Tiriti o Waitangi and recognise the richness of our diverse workforce.
  • Kaitiakitanga (stewardship) - we’re not a typical Primary Health Organisation! We exist to make it easier for people to stay healthy, and you can see this in the work we do to simplify access to primary healthcare and address equity barriers.
  • Haurora (health & wellbeing) - through flexible working, the offer of medical insurance, EAP services, and free flu vaccinations.

We’ve created an incredible working environment full of experienced and engaged people, and we’re looking for our next champion who identifies with our values to join us!

Mō tō mātou rōpū: About the team

Pinnacle has made an explicit commitment to Māori and our communities to improve equity of health and wellbeing, in accordance with Te Tiriti o Waitangi. We have also made an equity commitment to our Pasifika population.

Guided by a shift towards proactive care and with a focus on wellness, we think outside the box and we’re not afraid to innovate in our space! What we do each day is more than just a job, and that creates an energy which is contagious.
