Position Summary:  Responsible for supporting patient care, ensuring safety protocols are followed, improving performance, assisting in the continuity of care, and facilitating communication between patients and healthcare providers. This role requires attention to detail, strong communication skills, and a compassionate approach to patient care, with a focus on enhancing patient outcomes through careful management of care transitions, patient education, and collaborative teamwork. This position has responsibilities on both campuses and requires heavy involvement with the community.
