Description
****Average hourly is $16.63 (including wage, incentives, bonuses, overtime, shift differential, etc.)****
Position Summary
Provides direct support to families in the Family Preservation Program to promote child safety, stability, and well-being. Works collaboratively with case managers, therapists, and community partners to ensure families receive necessary services and meet case plan goals.
Key Responsibilities
- Provide daily direct and indirect support to families in home, school, and community settings
- Coordinate community resources (e.g., healthcare, childcare, housing, employment, benefits)
- Collaborate with case managers and therapists to implement case plans
- Offer emotional support, coaching, crisis intervention, and parenting guidance
- Assist families with accessing services such as SNAP, Medicaid, SSI, and employment support
- Maintain accurate and timely documentation and prepare records for audits
- Identify and address barriers to family engagement and service success
- Transport clients as required and attend court proceedings when applicable
- Support ongoing communication between families and service providers
- Promote teamwork, cultural sensitivity, and agency values
Qualifications
- High School diploma or GED required
- Valid driver’s license and ability to transport clients
